Progesterone, Testosterone, and Estrogen Production in Women

Women are born with somewhere between one and two million specialized follicular cells in their ovaries. By the time women reach puberty and begin to menstruate, only about 300,000 of these special follicular cells remain, and moving forward as many as 600 of them can work per month to produce estrogen, progesterone, and testosterone, and to compete to become an ovulated egg. These cells are completely used by the end of the month, and aren’t able to work in following months to produce hormones or to compete for ovulation. Women begin to suffer from symptoms of progesterone, testosterone, and estrogen deficiency as they get older, and there are less of these cells available to produce hormones.

Progesterone, Testosterone, and Estrogen Deficiency in Women

The by far most common severe sex hormone deficiency in younger women nowadays is progesterone. This is because anovulation has become relatively common. Whenever the final result of the monthly competition outlined above is that plenty of estrogen is produced, but for various reasons no ovulation happens, then progesterone isn’t produced during the second half of the month. If this happens often, women will suffer from various symptoms of progesterone deficiency, which will worsen over time. This ubiquitous syndrome is often misnamed “estrogen dominance” colloquially, although estrogen levels can be completely normal.

The second most common severe sex hormone deficiency in younger women nowadays is testosterone. Women are usually surprised to learn that severe testosterone deficiency happens earlier for them than it does for men. In women, testosterone production decreases gradually from age 21 and by age 40 women have less than half as much testosterone, and production drops significantly after menopause.

On the other hand, while women’s production of estrogen gradually declines from age 21, women aren’t usually symptomatic of severe estrogen deficiency until they’re getting to within several years of menopause – usually until around age 45 or so.

Most women will begin to obviously suffer from a combination of severe progesterone and testosterone deficiency, and mild-to-moderate estrogen deficiency between 40 and 50 years of age. It’s inevitable. For these women, this causes tremendous suffering and increased risk for serious chronic diseases.

Our Approach to Hormone Replacement Therapy for Women with Low Hormones in Portland, Oregon

We understand that the reference ranges labs use for progesterone, testosterone, and estrogen aren’t helpful. Reference ranges say that only 2.5% of women suffer from low progesterone, testosterone, and estrogen, and other hormones! Everyone on the planet Earth knows that almost all women experience tremendous suffering from the collapse of these hormones during perimenopauase and menopause. At the same time, they say that 2.5% of women have too much of these hormones. That sounds absurd because it is.

Reference ranges just consider all women together as though they’re all the same in terms of what levels of progesterone, testosterone, and estrogen are ideal for them, when we know that this is not the case. Some women feel and experience their best health with 10 times more or less of these hormones than others! What’s important and difficult to accomplish is to determine the ideal doses and levels of these hormones.

This is our area of expertise. It requires an in-depth understanding of how low progesterone, testosterone, and estrogen affects other hormones, medical conditions, genetic predispositions, and environmental factors, and it requires lots of experience. We have this understanding and experience.

Often Other Hormone Deficiencies Have to be Addressed As Women Age, Besides Progesterone, Testosterone, and Estrogen Deficiency

Low progesterone, testosterone, and estrogen due to normal aging, perimenopause, and menopause doesn’t happen in isolation. Often there are other hormone imbalances and deficiencies that also need to be identified and addressed. For example, the next most common hormone deficiency due to normal aging is by far growth hormone. After growth hormone deficiency, the next two most common hormone deficiencies due to normal aging are low cortisol and low thyroid hormone. These deficiencies are followed by low melatonin and low oxytocin, and then there are others.

We prescribe only bioidentical hormone replacement therapy, and we augment the prescription of progesterone, testosterone, and estrogen, and comprehensive hormone replacement therapy for women in general with the evidence-informed use of other medications, nutraceuticals, herbs, and realistic lifestyle recommendations, customized to your needs and abilities – so that we can work together to help you experience your full potential!
