Successful TRT for Men With Low Testosterone Requires the Expert Prescription of Three Medications

The foundation of successful testosterone replacement therapy (TRT) for men with low testosterone requires the expert prescription of the “Trifecta” of three TRT prescription medications: testosterone, anastrozole, and HCG, along with ongoing blood testing and follow-up.

Testosterone

We prescribe a dose of testosterone that gets you and keeps you within your normal and healthy ideal range, which means your average total and free testosterone levels are above the reference range – where they’re supposed to be!

Anastrozole

As men age their testosterone levels plummet and their cells decrease their number of testosterone receptors. The disappearance of testosterone receptors means that whatever testosterone is in the blood is more likely to be stuck in the blood outside of the cell, and therefore be converted into estrogen. To make matters worse, extra weight, especially in the mid-section, chronic stress, drinking a significant amount of alcohol, or using cannabis will all increase the conversion of testosterone to estrogen, and this whole process becomes a vicious positive feedforward cycle since higher estrogen levels will decrease testosterone production, causing testosterone levels to further drop, testosterone receptors to continue to disappear, and estrogen levels to further increase.

If men with low testosterone forego “Trifecta” TRT for low testosterone, then this disappearance of testosterone receptors gradually becomes irreversible beginning around age 50 or so, and worsens over the course of the next about 20-30 years, after which it’s mostly irreversible. In the mean time, it’s reversible, but it takes expertly prescribed short-term supersaturation of the testosterone receptors by testosterone for the cells to increase their number of testosterone receptors again. What this means is that when men with low testosterone initiate testosterone replacement therapy (TRT) they will initially require a significant amount of an aromatase inhibitor to prevent the conversion of testosterone to estrogen. Over time, the required dose of anastrozole usually decreases, but it can be quite significant for some time. Of course, in cases where men become a lot more physically active and improve their body composition, testosterone doses can increase such that the anastrozole dose remains the same or even increases. This is why it’s important to make sure you’re going to a specialty TRT clinic where you can receive testosterone replacement therapy under the supervision of a team of TRT doctors who are TRT experts.

If testosterone is prescribed without the appropriate anastrozole dose, then an overwhelming amount of estrogen will be produced from the prescribed testosterone, which will outcompete testosterone for testosterone receptors, and this pretty much causes the opposite of what men with low testosterone want from testosterone replacement therapy (TRT). Trust me, there are better ways for men to get in touch with their feminine side. Without the appropriate prescription of the just-right amount of anastrozole by a TRT doctor who is a TRT expert there’s no way to normalize testosterone levels without causing either overwhelming estrogen levels.

An example of how important estrogen levels are is that if men have too much estrogen, then they can’t achieve a firm erection, and if they have too little estrogen, then they can’t maintain one. I think that’s all I have to say about estrogen for men to understand it’s importance, but there’s a whole lot more to it than that!

Human Chorionic Gonadotropin

HCG looks just like the hormones LH and FSH that are made by your anterior pituitary gland in your brain to stimulate your testes to make testosterone and sperm, and estrogen, too, except it’s a lot stronger than LH and FSH. It’s exceedingly important that HCG is prescribed as part of a testosterone replacement therapy (TRT) regimen in order to prevent testicular shrinkage and a gradual loss of your ability to make testosterone and sperm, and estrogen, by your testes.

This is because when you are on a therapeutic dose of testosterone your total and free testosterone levels are in your normal and healthy, ideal ranges, so your pituitary gland turns off its production of the hormones LH and FSH, which it normally makes to stimulate your testes to make testosterone and sperm, and estrogen, because it figures that you have all the testosterone you need, so there’s no need to make more LH, and it figures if you’ve got all this testosterone around, then you should have plenty of sperm around as well, so there’s no need to make any more FSH either.

So essentially HCG is prescribed to replace LH and FSH. Studies have shown that 95+% of the functional capacity of the testes to make testosterone and sperm, and estrogen, is maintained with a therapeutic dose of HCG.

Also, because the penis is loaded with LH and FSH receptors, which also accept HCG, men are also even more likely to experience improved erectile function with the ongoing prescription of HCG. That’s all I had to write isn’t it? Now you understand the importance of HCG!
